Website design with online booking and payment integration + Logo/Theme design + Youtube Channel Art

Hi there,
I am looking for a full package from logo design to YouTube channel art to a website where users can do online booking and make payments for appointments.
Phase#1: Logo Design + Youtube Channel Art

Time: 2 days max
Details: In this phase, I would want to get a theme finalized, create a nice logo and get all my social art work done.
Deliverables:
a) Logo in all sizes and formats
b) Youtube channel art
c) Facebook page channel art
Phase#2: One page landing page/website

Time: 3~4 days max
Details:
In this phase, I would want to build a website using the same theme and logo designed in phase#1. Its a simple 1 pager website with booking facility. We can integrate calendly and stripe or any other service that you have worked on that can easily allow users to book appointments quickly.
Deliverables:
a) A nice, clean design 1 pager website
b) Booking via calendar integrated into the 1 pager website
c) Payment integration
d) A way for me to setup appointment slots and types of appointments.

1. I am currently a engineering manager at a tech company in Silicon Valley with over 15 years of experience in software engineering. Please talk technical stuff with me and get directly to the point. I will highly appreciate if you work in collaboration and ask for feedback regularly. I don't want us to be in a situation where you take a lot of time building something that I don't need.
2. Kindly only apply if you have previously worked on integrating booking and payment into website. I am NOT looking for a custom solution, I just want to integrate already existing solutions out there like calendly and stripe.
3. I already have the domain and can get any kind of hosting we will need for this.
4. This whole is around interviewing and mentoring service. I am working on my YouTube channel where I will give people free tips, advice and tricks to pass a specific type of software engineering interview called system design interview. The name of this project is "System Design Mentor", the logo and the theme and everything needs to reflect that. From YouTube channel, I want my audience to come to my website and book mentoring sessions with me. These sessions will be of 3 different types and I can provide the details when needed.
