Urgent Create 3 Custom Forms -- 2

I need 3 separate forms designed from scratch, saved both as high-resolution JPGs and print-ready PDFs. The finished artwork must be entirely original—different enough for me to copyright without conflict.

The look should be clean and modern, matching the visual style of my brand. I’ll supply my logo and any color guidelines you might need. Each form will contain clearly marked text fields only; no checkboxes or dropdowns this time.

Please lay out the pages so they can be filled in easily on screen and still look polished when printed. Vector or layered source files (AI, PSD, or similar) would be helpful for future updates, though the mandatory deliverables are:
• 3 final forms in JPG format (300 dpi)
• 3 matching PDFs (interactive if possible) and PowerPoint
