Template Design for Proposal of travel itinerary

I need a basic but sophisticated and simple design template for a Proposal Itinerary, It can be in Word or Excel - - I just need to be able to send it as a PDF. I need a template but it needs to be flexible so I can edit easily. I have a sample and would just like it to look professional. Don't need drop down menus - just a basic form that will look professional
