Support in building a form (templates are in German)

I have one form for which I need support for. Currently I have a template available in pdf from one of our competitors. I need this transformed into an editable word document. Our company's logo needs to be placed correctly and data input fields highlighted. Everything should be aligned to our corporate identity (font and style).
