Someone to build me a quiz that I can embed on company homepage

I need to create a good-looking quiz that will determine what asset tracking system will be best for ur customers. I want to ask what industry that they are in (Auto/Aerospace/Transit/General manufacturing/or Other) and two other questions and then, depending how they answer to spit out an answer of the type of system they should use along with a link to an appropriate page.

I want to embed this on our company homepage to help drive engagement, so it should look nice with nice buttons/icons etc.
