Simple landing page

We want a simple landing page for a free 1 day sports trial, want it to be attractive for social media marketing. All we want them to do is put their name, phone number , and email address and know what they are signing up for. This will be connected to our ads on sports consulting free 1 day trial. Need them to enter info, clean and simply. We already have a logo made, just need an attractive landing page to get us customers. we will provide logo for color scheme.
