• Ensure the form is easy to read and navigate.
• Use visual cues (e.g., headings, section dividers, or boxes) to clearly separate different sections such as “Child’s Details,” “Parent/Guardian Details,” “Emergency Contacts,” “Medical Information,” etc.
2. Modern and Professional Look:
• Aim for a clean, modern, and professional design that is visually appealing but not overwhelming.

• Consider using a light background color or subtle shading to differentiate sections.
3. Form Field Design:
