Key Points:
- The forms needed are Incident report forms, Hazard assessment forms, Safety training attendance forms, NORM Bulk Sampling Form, Noise Dosimetry Forms, etc.
- The design must be clean and professional, with our company logo incorporated.
- The layout should be easy-to-use and intuitive for our clients.
For the Incident Report Forms:
- They need to include fields for Date and time of incident, Location of incident, and a detailed Description of incident.
- There should also be space for Witness statements, as well as Measurements for designated substances.
- The incident report forms should be easy to fill out, yet comprehensive in capturing all necessary information.
