Please create a Power point design in din a4 and add some content

Hi, I am locking for someone who can create a power point slide that looks like the pdf I added ("Beispiel Anamnesebogen"). We have a different logo and some different content. The content i add should be place on the left side of the slide ("Fragebogen für Medikom). the rest should be the same as in the pdf mockup that has to be copied (("Beispiel Anamnesebogen")). please use our logo (Raute Logo Kardiologicum). You can cahnge the design if you think something else would be more suitable. It is a questionaire and i should be able to change the text. I would need it within 24h, thank you
