Photoshoot Voucher

Please design me a voucher to print and fill in by hand.
Double sides, so a cover one one side and a details to fill in section on the other side.
Must be able to fit 3 vouchers on an a4 paper.

The attached image does not have to be included, but could work nicely.
See logo attached.
