Packaging for sampling of a dice game

I want to create a small packaging (2cm x 2cm x 5cm) and will make the die cut for it available. Though, I need someone to create the artwork based on the following brief (see packaging sampling.pptx)
All additional files may be used for the creation of the packaging.
