PDF document - Patient referral form & flyer
Budget: $30 – $250 USD
We need 2 pdf documents for our dental clinic:
(1) a pdf form, 1-page A4 front and back, needs to be digitally fulfilled and printable (see attachment 1)
(2) a pdf flyer with patient information and a map, 1-page A4 front, (see attachment 2)
Examples are attached. Our document n.1 will present up to 40% more text and fields
The logo, the style guideline (colors and font) and the texts will be provided.
Please provide the (1) the price, (2) when you could start and (3) the duration for this project.
In the case of good collaboration and solid result, a higher-volume subsequent phase of work might be planned/done together.
