Need book cover designed

I need an illustration of someone scared standing on a grave being injected with poison. That is how I want my book cover. The title of the book also needs to be incorporated into the illustration as well.

Do NOT have the same layout as

Top: Book Title
Middle: Image
Lower: Subtitle
Bottom: Author name.

Do something different. Looking for original layout.
