Mail Chimp Landing Page for Database Collection

I'm seeking a professional to create a Mail Chimp landing page that will serve as a Digital Connect/Reconnect Card. The primary goal of this landing page is to collect data from users.

The Digital Connect/Reconnect Card will need to capture the following information:
- Name and email address
- Phone number
- Mailing address
- V card
- Subscription to a weekly email list about ongoing events
- Requests for clergy contact
- Interest in additional information about worship, outreach, care teams, and studies

One side of the card will be for 'CONNECT' while the opposing side will be for 'UPDATE/RECONNECT'.
