Logo and Greeting Card Template

I am seeking an estimate for the following:
1. Create a fairly simple logo, of which I have some design ideas already. It will be faintly simple.
2. Create a Photoshop Template that I can use to print greeting cards. I’ll have a photo on the front. Logo, QR Code, etc on the back, and possibly some simple artwork on the inside as a border of the inside pages.
