Label Design for a Supplement Buisness

I need a label design for Creatine Monohydrate 500 grams 17.6oz. I have a sample of how I'd like the label to come out. The actual Jar will be black. I'd like the label to be half white half black like the sample image but reverse. The size of the label is 4.75" x 10". It will be needed in AI Format and live sample images will be great.
