Interactive and drop-down options for a word template

Hello, I'm looking for a freelancer to create an interactive word template for me. I need to combine several different word templates into one, where anyone can tailor their document depending on the options they select, which would auto-populate the approved sales verbiage we have. Specifically, I need a template with four to seven drop-down options, and text input fields. I don't have a particular design in mind, so I'm looking for help with the actual design. I'm looking for someone who can provide creative and unique ideas to make this template interesting and attractive. Ideally, the freelancer should be able to make this template professional and efficient, while still being aesthetically pleasing.
