Input my design onto dieline

This is a very simple project if you know how to use illustrator, but I do not know how to use illustrator. I need you to input my design onto the dieline provided. I have mockups which explains where everything needs to go, and I have the images that need to get placed properly everywhere.

I need this done as soon as possible. If you can do it right now, that's perfect.
