Homemade label -- 2

I’ve made some homemade vanilla and want to give it as a gift to friends and family. I want a label for that vanilla (which will go on an amber-colored bottle- pic attached). My name is Olivia so cool if that can be incorporated (Livia’s Vanilla?) but not a must. I'd like for an image to be part of the text (eg something like the "v" in vanilla spelled with vanilla beans... just an idea). I’d love a blank spot to say whether it was mixed with bourbon or vodka and the year. It’s Madagascar vanilla beans.

See photo-- it's a 4oz / 120mL bottle, so I need a file that can be printed and fit on that bottle.
