Homemade label

I’ve made some homemade vanilla and want to give it as a gift to friends and family. I just want a label for that vanilla (which will go on an amber-colored bottle- pic attached). My name is Olivia so cool if that can be incorporated (Livia’s Vanilla?) but not a must. I’d love a blank spot to say whether it was mixed with bourbon or vodka and the year. It’s Madagascar vanilla beans.
