Generate a good looking design for a template document

Hi! I'm looking for a freelancer with previous experience with image and documentation design (if you have proven work developing templates, even better).
The main objective of this project is to generate a design that beautifies the document that you will find attached to this post (a screenshot of the document that we should work with, after getting the project i'll send you the editable), a few references and a also a white version of freelancer's logo that should also be used in the final design.
The more minimalist but professional, the better.
The delivery format should be an A4 size PDF document.
