Fillable Financial Planning Factfinder

I subscribe to a financial planning program that has their branded factfinder. I would like to create a similar factfinder with my own branding and slightly different questions. I have attached the original and a rough draft mockup. Looking for a professional piece which is both printable and fillable on the computer. I do not have the skills to perfect this. Thank you!
