I need a single, well-structured clinical logbook that nursing students can carry through every rotation. Its central thread is the learning objectives for each placement, so every page, checklist, and reflection space has to circle back to measurable progress against those objectives.

Content requirements
• Patient data section that captures demographics, diagnosis, and a running chronological note area stamped with date, time, and clinical condition.
• Detailed clinical case records, followed by a dedicated narrative reflection block after each case or procedure.
• Nursing-procedure checklists (vital signs, medication administration, wound care, etc.) with tick-boxes for completion plus space for comments on technique.
• Medication administration table that forces entry of dose, route, and exact time.
• Free-comment area for insights, questions, and unexpected events.
• Tutor signature lines and end-of-shift evaluations.
• Structured assessment forms built around clear criteria, a scoring system, and tutor feedback sections so grading is transparent.
• Guided examples of “model” clinical cases and quick references to best-practice guidelines to show students the level of detail expected.

Format & usability
