Editable PSD Document Creation

I'm looking to have a PSD document designed which I can edit. It should allow me to easily change text elements (like name and address).

Ideal Skills:
- Proficient in Adobe Photoshop
- Experienced in graphic design
- Familiarity with creating editable PSD files
