Digital Design for Internal Company ID Card

We are looking for a freelancer to recreate a design for our internal company ID cards. We already have a template, and it needs to be digitized so that we can easily edit and update names when needed.
Requirements:
• Accurately recreate the provided design
• Ensure that the name field is editable for future modifications
• Deliver the final design in a fully editable format (PSD, AI, or similar)
