Design me a loyalty card for my laundromat

We use purchasable, reloadable loyalty cards at our business. The cards are credit cards that are used to start all of our washers and dryers. With our new logo (will be supplied), we need to design a new laundry card to rebrand our business.

I will provide the current laundry card for reference purposes, all information needed on the new card, and the new logo needed to be on the laundry card.
