Design and format my book for Kindle

I'm looking for a high quality book design for my upcoming release.The subject of the book is survival and wilderness living. Please don't respond to this posting if you don't meet the requirements.You must have book designs in your portfolio and you must be a very experienced graphic designer.

What I need:

- Book cover design (front and back)
- Book pages and chapters designed
- Print files (Adobe PSD and Adobe illustrator)
- Kindle formatted
