Design digital PDF file

Hey guys, I need a stylish and modern looking PDF file that can be opened on any pc, filled out digitally and, in best case, be signed and sent via email when completed. If any questions, please ask. It needs to show some icons and a logo as well. I have a sample i have uploaded. design should be similar. Logo attached as well. Exact text can provide...problem.
