Design Two PDF Forms

Two (2) fillable, PDF forms needed. Description for second form will be provided after first one is complete to lessen the complexity.
Description: Please use the same colors, fonts, and style as in the attached Talent Assessment_FINAL document. The new PDF form will be based on the attached Example document. It is not scaled to size given I drew the outline. The Sample document is simply an illustration of what the boxes shown on the Example document should be. The text on the Sample document may be disregarded. *This project will require some back and forth for edits* I appreciate your patience. The second document, a part of this project, will be based on the first one created.
Given the Example document is not true to size, the ratings scales shown on the Example document should be side by side with the Action box shown underneath both areas (large comment box).
