Customized Word Document - Business template

Hi there,

I'm looking for someone to assist in helping me create a customized word document (1-2 pages). I can provide my business logo and I've included below some sample images.

I'd like for there to be a customized border including astrological symbols/signs. I'm not really looking for a brochure or anything of that sort as I'll create the different pages respectively (such as table of contents etc.).

I'd just like a unique design template I can use when providing a written service to others. Happy to provide my website as well as more information about the services provided.
