Customer Satisfaction Survey Design

I need a customer satisfaction survey designed for my company.
- Purpose: The survey will serve to Measure customer satisfaction, Identify areas for improvement, and Gather feedback for product development.
- Target Audience: I want the survey to be distributed to both New customers and Existing customers.
- Distribution: I plan to distribute the survey in person.
I'm looking for a professional, printable survey design that resonates well with the target audience and is easy to understand. The survey should be visually appealing and should have a good balance of open-ended and close-ended questions. I want to be able to fit two surveys on a page, so I can cut the page in half and have two. I have uploaded the initial template to go off of for questions and fields to use.
We are a Jewelry Appraisal company with a full laboratory. We identify and appraisal all jewelry, gemstones, and watches.
The chosen freelancer should ideally have experience with:

- Experience in both online and offline survey distribution.
