Create a form for verification

Create a form for numerical weather model verification that is easy to use as a hard copy and soft copy. I don't have many restrictions so you can be as creative as you want. I want to have hard copies that will be available to forecasters in the forecasting office or soft copies to be added and sent automatically (ex: a link that will send the forms automatically to my email).
