Create a MS Word eBook template (and associated Marketing "book" images)

I am looking to have you do the following: 1) create (using the attached design draft) a MS Word-based eBook template that is comprised of a title page, 2nd page, and back page 2) an image of the eBook title page that looks like a real book (w/3D dimension to it) to place on a "landing page" (webpage). Regarding #2, I would like two (2) different sizes of "the book" image - a more prominent dimensioned image appropriate for use on a web page and a smaller dimensioned image for other Marketing uses (e.g, a posting on LinkedIn.com)
