Create a Standard Customer Enrollment Form (Based on a template)
Budget: $25 – $50 AUD
We are having a revamp of our client facing branding, styles and forms, and we are looking for an experienced graphics designer who could create a new standard enrollment form for us. You do not need to invent anything from scratch as we have a prototype design we would like to base the form on - we will supply high resolution images and text once the contract is awarded. There's a mixture of English and European letters in it, plus some shapes with about 5 base colours in it and some basic drawing. This is a 4 page A5 booklet, which we will need to be delivered as a pdf so we could supply it to our printers here.
We anticipate that this basic piece of work would take an experienced designer no more than 2 hours, and we are offering AU$50 per hour for this.
REQUIREMENTS, please supply us with:
- at least one example of similar work done previously so we can ascertain the quality of your output (you can blur out any brand names, send us a private message with images, etc). As a pdf/png/etc
- the timeframe for completion (we need this to be finalized within 48 hours, and 99% of the work done within the first 24 hours, to spend the rest on any refinement work if required)
- the cost - we will select predominantly based on the budget
