Create a Microsoft Form Template

I am looking for an experienced Microsoft form designer who can create a template for me. This template should be in the form of text boxes and must be flexible enough to be potentially embedded in a web page. I have existing text and images that I would like the designer to use in the form, if needed. If you have experience in designing forms for Microsoft systems, then I would love to hear from you.
