Create Labels for shipping

I want to create labels with specific backround. This label will be for shipping so the background will have to be able to print text on it such as name address etcetera. Here is where you can get the template from the manufacturer. https://www.avery.com/templates/5163

Here is the logo, once again I will need to be able to print addresses on these labels.
