Create a Flowchart for Survey Research article

Use a graphic software to create a flowchart for original research article. The article is a survey of medical student in Baghdad. The journal has requested that a flowchart should be produced which describes the sampling process and method in graphical formant :

The flowchart should show the sampling frame from which the subjects were selected, reasons for non-inclusions, and the numbers and reasons subjects drop out during the course of the study.
