Create Editable PDF Forms

I have a series of existing forms that only exist as non-editable PDFs. I need each one rebuilt so users can type directly into every field while preserving the exact look and layout of the original.

Scope
• About one form will come your way every two days.
• Each form must be recreated as a fillable PDF with correctly labelled text fields, check-boxes, radio buttons, or drop-downs wherever the source indicates.
• Visual fidelity is important—fonts, spacing, logos, and page breaks should match the original document.

Deliverables for every form
1. A fully editable PDF ready for distribution.
2. The working file (e.g., Adobe Acrobat Pro, InDesign, or similar) so future tweaks are easy.
