Business Document Templates Creation

I'm seeking a freelancer who can expertly design two types of business document templates; specifically for allied health reports and intake forms. I am rebranding my business, I have a new logo and colour scheme in mind. You'll need to create a standard yet professional format which includes:
- Headers and footers that will prominently display the company logo and name
- Tables within reports to reflect assessment data
- Fillable PDF Intake form
Ideal candidates should have a keen eye for detail, an understanding of corporate identity and branding, plus extensive experience in graphic design. Proven experience working with business document templates is a must. I also have examples from my old business templates which has all the information I require, I just need the forms to be converted into a more modern look with the new colour scheme that aligns with my new logo.
