Build me an online form
Budget: $30 – $250 CAD
I have a PDF of a form we use in our business. we usually
print it out and hand write in the check boxes. I want to
change that form to an online form to be used as a template
and then edited each time we need it. Attached is what we
use now (Plus there may some items added) Im ok if the
design of the form changes a bit but the content is what we
want
