Branded PDF Form Design

I require a professional to create a PDF document containing text, my company logo, and multiple selectable checkboxes. I own a high-resolution version of my company's logo for use in the design. The main thing to solve here is clean design and selectable checkboxes. PDF will have 12-13 pages where each will have 20-ish checkboxes.
