Birth Certificate Redesign

I need a fully editable birth certificate designed to look like the attached picture.

Key Requirements:
- Must be fully editable
- Should include fields for the pets full name, date of birth, place of birth, gender, and parent's names
As well as the other words on the attached picture . I want everything that is on the picture fully editable
Timeframe:
- The completion of this project is urgent and needs to be done ASAP
