FY 2023 Navigator Grant Program Request for Proposals
Budget: $250 – $750 USD
I need help of writing (3000-5000 word count) to apply MNsure FY 2023 Grant for my organization describing the need of services my organization provides to the communities. Describing specific populations and/or geographic area (cities, counties, & regions) where my organization have an office to provide services in the community due to their circumstances (Immigrant, language, technology and other etc barriers).
the grant Letter is to highlight the needs and services we provide certain populations that our Organization works with and intends to serve the grant money some the barriers in community by outreach services.
this is where you elaborate specialized outreach or assistance for population facing barriers within the geographic area
Describing geographic area that will be served by the grant (cities, urban region) highly density, poor communities who face barriers getting access to technology due to their limit of education
this grant will allow these particular communities to give access to navigator services
These three (Geographic (focus on geographic coverage) Population (focus on enrolling specific populations) and Capacity Building (focus on navigator organizational development) are the main pillars where organization can provide services to the communities
Mainly my populations are immigrants, communities of color, urban living communities facing barriers such as language barriers who have minimum or lack of education to access some of the healthcare services provided by government agencies
that's when my organization coming and help the communities to serve those areas where they struggle to do their own
such as Navigating Healthcare Applications based on Govt online
accessing their primary account by calling the healthcare Dept and retrieving client HC information
Explain type different programs based on client household income, immigration, and verifications
My organization has been serving last 3 years almost over 3000 clients (insuralink) however during the covid19 pandemic has put us in difficult situation and has slowed down on our navigator services for shutting down all walk -ins client and our reach services. currently we only assist tru phone call services.
predominately serve many communities including East African Communities (Somali, Oroma, Liberian) and Latino Communities from (Mexico, El Salvador and Honduras)
We helped them explain the different type of Healthcare Program provide by the state and the counties and help them enroll the process online on the Minnesota State market place

3 years ago we started with the organization (Insuralink LLC) with one employee and hired 8 more additional employee who are bilingual or tr-lingual to close the barriers facing in our community such as language, and help navigate computer applications, renewal, verification ets to ease the lack of computer and understanding the system
