Populate Google Doc with Google Spreadsheet

I have a Google Sheet that has jobsite information. I need a button or link for each record in the Sheet that will open a Google Doc and pre-fill that Doc with information from the Sheet. This is different than a Mail Merge as I want just one record to create one doc at a time, that's why I envision that it would be a separate button or link for each record that would run the script for that one Sheet record. If this can be done with a URL link, even better. I just can't figure out how to create Doc fields that can be referenced in the URL. (The current "Create Ticket" link you'll see in the screen shot does this, but links to a Formstack page where the form fields are easily identified in the URL link.)
