NDIS Bulk Billing Spreadsheet

I need a clean, reliable spreadsheet that lets me generate NDIS bulk-billing files quickly. It must capture three core data points—client details, billing amounts and service dates—and rely on CSV import as the only data-entry method, so the layouts, headings and any validation rules have to align perfectly with a standard comma-separated file.

Once the CSV is brought in, I want the sheet to organise each record clearly, keep formats consistent (e.g., date fields as dd/mm/yyyy, currency as $0.00) and be ready for the usual billing checks, such as confirming a participant’s name against their NDIS number and spotting duplicated service dates. If simple totals or cross-checks help streamline my bulk upload to the NDIS portal, build those in; just keep the file fast and uncluttered.
