Google Forms for Data Collection

I need someone to assist me in creating a Google Form that can be used as an input document for various types of data. Key requirements include:
- Designing the form to include text fields, checkbox fields, and date fields.
- Setting up the form to directly transfer the data to a Google Spreadsheet as input is received.
- Implementing automation for email notifications. This will help us stay informed when new data is collected.
Ideal candidates should have experience with creating Google Forms, working with Google Spreadsheets, and setting up email notification systems. Knowledge of data collection best practices is a plus.
