Google Form feed to a Google Doc

This project is for a Non-Profit Food Pantry, each client has a scannable card that they present for services. Upon arrival, the receptionist scans a bar code on their card and their name and information pops up on the screen (A Google Form) and the client is asked to verify the information and changes can be made. The Form then feeds/updates the client information in a Google Doc
Currently, in our efforts to build this platform, we can't get the client information for the 2nd, 3rd, etc. client to advance to a new line in the Google Doc, each subsequent client is overwriting the previous one.
We need your expertise to address this.
