Google Docs–Forms Automation

I have 5 patient-facing Google Docs that need to become streamlined Google Forms so I can gather data quickly and feed it straight into a central Google Sheet. The form must capture contact information as well as medical demographics, then automatically populate selected Google Doc templates with that same data.
Here is the exact workflow I need built:
• Users open a Google Form generated from my existing Docs.
• They enter contact information and medical demographics.
• Names, addresses, and other medical information autofill where appropriate so the user never types the same fact twice.
• When the form is submitted, Apps Script (or any suitable no-code add-on you suggest) writes the collected values into specific merge fields inside several Google Docs I already use for intake.
• A scribble-style signature box appears at the end of the form so patients can sign on phones or tablets, and that signature is embedded back into the completed Google Docs.
I will share the current Docs, the target Sheet, and a short list of the fields that map to each template the moment we start. The end product should leave me with:
– A clean Google Form for every document.
– A single, well-structured Google Sheet receiving each submission.
– Updated Google Docs that arrive fully populated—including the captured signature—ready for printing or secure emailing.
