Extract MCQs & Answers from Video

I have an MP4 recording that runs through a series of multiple-choice questions. I need every question captured verbatim along with its correct answer, then delivered in a clean, easy-to-read sheet (Google Sheets or pdf file or Excel is fine—whatever you prefer).
The video contains only multiple-choice items, so no true/false or open-ended text needs to be handled. I’m after an accurate transcription of the wording, option labels (A, B, C, D, etc. if shown), and a clear indication of the right answer for each item.
Deliverables
• A spreadsheet with separate columns for question, each option, and answer key.
• All content taken directly from the MP4; please do not paraphrase.
