Digitize PDF Survey Responses

I need to turn a set of survey responses, now stored in digital PDF forms, into a clean, structured spreadsheet. The end goal is straightforward data entry so I can run quick analyses later.
Scope
• Open each PDF and capture every answer exactly as written.
• Enter the data into Excel or Google Sheets, using clear column headers (one column per survey question).
• Flag illegible or missing answers in a separate “Notes” column so nothing is lost.
• Deliver the completed file and all working documents.
